Lesson Sign Up Form- PLEASE remember 6 lessons per semester. If you want more than 6, that is great!

Please note that you can come in with someone else from your section if you would like. You do not need a lesson with me if you have private lessons outside of school.

You are more than welcome to come in for extra help if you would like. Those without private lessons outside of school need to have 6 lessons with me per Semester.
